Clinical Presentation: Acute flank pain on theright
Comment: The hypovascularity on CDFI and CT of the renalparenchyma is
related to the ischemic vascular reaction related toinfection. Often there is an enhancing
peripheral rim (thehomogeneity depends on the degree of liquefaction which hasoccured.)
Differential diagnosis of the cystic rean masses include:A) Unilocular cystic mass 1) Simple
or Complicated (hemorrhagic orinfected) 2) Focal hydronephrosis 3) Caliceal diverticulum
4)Mature abscess 5) Arteriovenous malformation B) Multilocular cysticmass 1) Multilocular
cystic nephroma 2) Multicystic adenocarcinoma8) Localized cystic disease CT generally
provides better overallinformation in determining the presence and extension ofperinephric
abscess. US is excellent for the follow-up of abscessestreated conservatively.
Description: The right kidney is increased in size with a 1.5 cmthick walled
hypoechoic complex mass of the upper pole. Internalmobile debris were seen. The CDFI
didn't show any flow within themass but revealed reduced vascularity of the cortex of the
superioraspect of the kidney.
