Clinical Presentation: Annual check-up of an aorto-bifemoral bypassgraft
undertaken 8 years prior.
Comment: Postoperative graft surveillance is of particularimportance because
flow impairment is the major cause of graftfailure. Duplex Sonography is a good
non-invasive test fordetermining graft patency and can locate stenosis
accurately.Limitations include identification of collateral blood flow anddependant on the
skill of the operator. 3D MR angiography can bethe next test.
Description: In the B mode study the prothesis is hyperechoic witha sieve like
surface. The bypass is permeable and the spectralstudy shows an abrupt systolic
component consistent with arelatively noncompliant nature of the bypass material. The
femoralpole of the bypass is ectatic suggesting that the abdominal aorticaneurysm
extended into the femoral vessels.
