Clinical Presentation: Workup of right lower quadrantpain.
Comment: The normal appendix is not visualized in most cases. Adiameter of
greater than 6 mm, noncompressibility of the tubularstructure, lack of peristalsis, or
presence of a periappendicealfluid collection characterizes an appendicitis on US.
USsensitivity is about 85-90% and US specificity is about 90-95%. IfUS is negative, you
should consider to continue investigation witha CT.
Description: By using the graded compression technique, sonographyshows an
enlarged(>6mm) and thickened(wall >3mm)appendix andCDFI demonstrates hyperemia of
the wall. CT perfectly identifiesthe inflamed tubular structure within the right iliac fossa
withoutany sign of perforation. Surgery confirmed the diagnosis of acuteappendicitis.
