Clinical Presentation: Acute testicular pain
Comment: High grade testicular microlithiasis(II or III) can beassociated with
testicular tumor. Follow up study with US at 3months is warranted. In cases of equivocal
findings MR can beperformed. The main differential of acute scrotal pain istesticular
torsion with infarction. Epididymitis is the most commoncause of an acute painful scrotum.
When torsion is diagnosed in thefirst 5-6 h from the onset of pain testicules salvage may
be80%-100%. This rapidly decreases to only 20 % at 12 h. CDFI andPower Doppler show
decreased or absent flow.
Description: The right testicle is of normal size presentingseveral microlithiasis but
no sign of inflammation. The epididymisis enlarged, hypervascular and presents small
cystic areas whichcould represent small abscesses in the clinicalsetting.
