Clinical Presentation: Patient with history of severe alcohol abuse.Presents with
severe hepatic insufficiency. Rule outhepatocarcinoma.
Comment: The widespread adoption of ultrasound for screeninghigh risk patients,
has led to the identification of many patientswith a potentially treatable hepatocellular
carcinoma. US is thescreening and surveillance modality of choice. The serum
AlphaFoeta Protein (AFP) is much less sensitive.
Description: The liver demonstates right lobar artophy as well aslobulated
contours and ascites is present. The echo-pattern of theparenchyma is inhomogenous
and a hypoechoic nodule is present inhepatic segment VI. The doppler scan shows a
vascular pedicleentering the nodule. Spectral analysis demonstrates an arterialwaveform.
A contrast enhanced CT confirms the doppler study and thesurgical correlation was a well
