Clinical Presentation: Work-up of alcoholiccirrhosis.
Comment: The widespread adoption of ultrasound for screeninghigh risk patients,
has led to the identification of many patientswith a potentially treatable hepatocellular
carcinoma. US is thescreening and surveillance modality of choice. The serum
AlphaFoeta Protein (AFP) is much less sensitive.
Description: US shows a diffusely hyperechoic and inhomogenousechostucture
of the liver and a 1 cm hypoechoic mass lesion in theright lobe(VII segment). CDFI reveals
a central arterial pedicle.CT and angiogaphy demonstrate a focal hypervasclar lesion in
thearterial phase becoming isodense in the venous phase and onangiogaphy the lesion
displays a tumoral flush. The portal vein waspatent. These findings are consistent with
hepatocarcinoma whichwas confirmed surgically.
