Chapter: Liver & Spleen
Clinical Presentation: Work-up prior to hepatic transplantation.Viral cirrhosis.
Comment: The widespread adoption of ultrasound for screeninghigh risk patients,
has led to the identification of many patientswith a potentially treatable hepatocellular
carcinoma. US is thescreening and surveillance modality of choice. The serum
AlphaFoeta Protein (AFP) is much less sensitive.
Description: The US scan shows signs of hepatic cirrhosis with 2focal lesions
located in the hepatic segments VI and VII. These arehyperechoic, well defined and
present a central hpoechoic portionwhich demonstrates arterial flow on Doppler flow
imaging. Theselesions are suspect of multifocal hepatocarcinoma which wasconfirmed by
contrast enhanced CT and digital substractionangiography.
