Clinical Presentation: Routine work-up of a combined renal andpancreatic graft.
Comment: The only established role of sonography is to detectperipancreatic fluid
collections (lymphocele), pancreaticenlargement and to guide fluid aspiration. Color
Doppler Flowexplores the vascular anastomosis which are at risk for thrombosisin the early
postoperative period. Detecting high RI > 0.7 arenot sensitive nor specific for diagnosing
early transplantrejection.
Description: The CT and ultrasound study shows that the renal graftin the left iliac
fossa has a normal appearance and that thearterial and venous anastomosis are
permeable. Moreover the graftis homogenously perfused on the Power Doppler image and
theinterlobular vessels are well seen. The CT study displays similarfindings. Note that the
vascular anastomosis are with the iliacvessels. A small lymphocele is present. The
pancreatic graft isperfectly seen in the right iliac fossa. The vascular anastomosisare
normal as demonstrated by the Color Doppler findings. It ishomogenous and it is well
