Clinical Presentation: Erectile dysfunction
Comment: Erectile dysfunction can have many causes. Insufficientarteial flow is
typified by peak systolic velocities in the CCartery of <35 cm/sec after intracavernosal
injection ofprostaglandins. Venous insufficiency or leak is characterized byend diastolic
velocities of> 5 cm/sec. Erection is present whenthe diastolic flow is inversed as shown
by the presentcase.
Description: The US scan did not reveal any abnormality of thecorpus
cavernosum(CC). 20 mg of a PGE1 was injected in the rightCCA and we noted a perfectly
normal hemodynamic response with peaksystolic velocities in the arteries of the CC of >35
cm/sec.Flow inversion was noted in the diastole confirming the integrityof the
veno-occlusive mechanism. This is well typified byalternating blue and red signals in the
CC arteries on the dopplerscan. The patient presented an erection during thestudy.
