Clinical Presentation: Aasthenia and generalfatigue.
Comment: Color Doppler US was crucial in the differentialdiagnosis of the
pancreatic body cystic mass which was confirmed tobe a splenic artery pseudoaneurysm
and not a pancreaticpseudocyst.
Description: The ultrasound study reveals a cystic mass at thelevel of the
pancreatic body which at Color Doppler demonstratedhighly turbulent arterial flow. This
was consistent with a splenicartery pseudoaneurysm and excluded the diagnosis of a
pancreaticpseudocyst. The etiolgy of the pseudoaneurysm was not clear but waspossibly
due to recurrent pancreatitis although at CT the pancreaswas normal. The spleen
presented several hypoechoic solid lesionspresenting central arterial flow. A hypoechoic
lesion was presentin the liver and percutaneous biopsy revealed a
adenocarcinoma(metastasis from a colic cancer). The splenic lesions were alsothought to
be secondary lesions. The Color Doppler US findings werecorroborated by contrast
enhanced CT and digital substractionangiography. The patient refused therapy(
percutaneous embolizationof the splenic artery aneurysm).
