Clinical Presentation: Fever and abdominal pain in a patient withCrohnís disease.
Comment: Liver abscess can arise from pyogenic and fungalinfection.
Pathologically, abscess have necrotic centers, oftenwith liquefaction and are surrounded
by hypervascular rims. Theclinical scenario in this patient was in keeping with a
hepaticabscess but the differential diagnosis includes metastasis with atarget like aspect
on the CT scan.
Description: The ultrasound scan reveals a hypoechoic mass lesionwith multiple
central echoes and posterior enhancement in the rightlobe of the liver. The Doppler study
demonstrates peripheralhypervasculaity and no central flow. The CT scan shows a
hypodenselesion with rim enhancement. Aspiration biopsy confirmed thediagnosis of a
pyogenic liver abscess. Subsequently, the patientmade an unremarkeable recovery.
