Clinical Presentation: Work up of syncope in a patient with nohistory of smoking,
coronary artery disease or peripheral vasculardisease. The past medical history was
significant for laryngealcarcinoma treated approximately seven years ago with surgery
andexternal therapeutic neck irradiation.
Comment: Radiation induced vasculopathy is well known. Patientsthat undergo
neck irradiation are at a five-fold increased riskcompared to non-irradiated patients of
developing moderate tosevere carotid artery stenosis (Moritz arch surg1990).
Description: The history of radiation therapy prompted a dopplerultrasound
evaluation of the carotid arteries. The doppler scanrevealed concentric hypoechoic low
grade stenosis (aprox 40%) ofthe right carotid bulb and the origin of the internal
carotidartery. The digital substraction angiography confirmed the Dopplerfindings.
